Loving Hearts Energy Assistance Funds come from several different sources. All programs sponsored by Loving Hearts Outreach will follow the same rules and regulations of Loving Hearts Outreach. The following documentation must be provided before your application is processed. Please allow 3-5 working days for processing once all documentation is in order.
A Client may receive one energy assistance grant per year per household. This is valid while funds are available and to participating vendors only. All required documentation must be in order and up to date, and client must meet the LHO Financial Guidelines. Client must be a resident of Franklin County, Missouri.
1. Client must apply in person at Loving Hearts Outreach, 1902 West Main Street, Washington MO. Client must complete an application for energy assistance at that time.
2. Disconnect Notice from Utility Company. Utility Account must be in the name of Client applying for assistance. Account may be in spouse’s name as long as that spouse is living within the home. Please DO NOT wait until the day before your disconnect to apply for assistance. Processing of your application can take 3 to 5 business days to complete.
3. Valid Missouri Picture I.D. on each adult member of household. This is anyone 18 years or older living within the home, whether related or not. If a person is listed on bill that is no longer in home, proof of current residence is needed, this can be either a lease agreement, a mortgage receipt or an electric bill in that persons name for current address.
4. Social Security Cards on all persons living within the home. Court related documents, providing temporary or permanent custody, must be presented if a child other than your own is living within your household
5. Proof of one full month’s income on each adult member of household 18 years or older. This must be the month prior to applying for assistance. Proof of any other income to household including Grants, Foster Care and/ or Adoption Subsidies, and Child Support must also be provided. Documentation on child support must be provided by all single parent households. The following will be accepted as proof of income:
a. one month’s paycheck stubs
b. Documentation on company letterhead stating the number of hours worked per week and the amount paid per hour. This must also contain the name of the client, and a phone number and contact person for the company.
c. Most recent SSI, Social Security Disability Benefit Statement and/ or Social Security Death Benefit Statement from the Social Security Administration.
d. Documentation that client is currently registered at Missouri Career Center (this must be turned in for each client that has no income from work or Social Security).
6. Loving Hearts Outreach will need a copy of a client’s LIHEAP Grant Letter or a referral from JFCA (If applying October 1 through March 31 only ). |
